The "Assignment" portion needs to be complete and signed by the beneficiary, the "re-assignment" should be signed by the funeral director and notarized.
The "Small Estate Affidavit" should be completed if the primary and contingent beneficiary's are deceased or NO beneficiary is listed
Depending on the insurance company a Lost Policy Affidavit should be completed if the original policy is lost or destroyed.
