The Process  1-2-3

1. Complete  the Assignment & Re-Assignment form to be sent to  AFF for policy verification and acceptance. This can be done by Direct Upload, fax, or email.

2. Upon verification of the policy status, AFF will notify the funeral home of acceptance and funds will be disbursed immediately.

3. Mail, Fax or Email paperwork needed to finalize the claim with the insurance company.
